Last-long workpackages

Work package 1 (WP1) aims to ensure good organisation, quality and the achievement of goals for the project. 

Work package 3 (WP3) intends to investigate effectiveness and feasibility implementing patient pathways for stroke survivors after discharge from hospital and up to 3 months after the stroke. We want to find out whether the introduction of patient pathways leads to better function in the stroke survivors. We are further interested in investigating whether the implementation of the patient pathways leads to increased patient satisfaction and if there is a  link between patient pathway adherence and self-perceived health. WP3 is led by department's senior physician Bente Thommessen at Akershus University hospital.

Work package 4 (WP4) intends to evaluate the effect of an individually and multifactorial long-term follow-up by a health coordinator with focus on different domains. Those are physical activity and exercise, cognitive and social stimulation, optimal secondary prevention and other lifestyle factors. In order to prevent functional impairment, we want to answer: Will long-term and regular follow-up by a health coordinator prevent loss of function after a stroke? Will long-term and regular follow-up by a health coordinator prevent new strokes? Does long-term and regular follow-up by a health coordinator lead to better self-perceived health? WP4 is led by Professor Torunn Askim

Work package 6 (WP6) deals with the publication of the results from LAST-long. Our primary goal is to develope a publication and implementation strategy to spread knowledge about the project to researchers, politicians, the general population and people with stroke in particular. WP6 is led by Professor Ingvild Saltvedt
